Credit Rescue Program Enrollment (by: Five Star Credit Solutions) PART ONE

Welcome to Five Star Credit Solutions, LLC, and congratulations on taking your first step towards your Credit Freedom. Thank you for selecting us to assist you with your credit restoration and credit score improvement needs. Our goal is to provide you with exceptional service, and assist with removing inaccurate and timely information. Before we can get started on your file, please complete PART'S 1 and 2 the enrollment process which are further explained below.

Your card WILL NOT be charged until the completion of your consultation call.

Step 1: Customer Information

(STEP 2: Credit Monitoring)

It is imperative that you purchase your credit monitoring by the consultation date. As a client of Five Star Credit, your are REQUIRED to obtain and maintain monthly credit monitoring services from our PREFERRED provider. The cost will be $24.99/m.
The credit monitoring provider is IDENTITY IQ. CLICK HERE to get a copy of your report. Credit monitoring is required so we can pull your report monthly and see what comes off and on your report. Once you obtain your credit monitoring please place your credentials in the box fields below.

Monitoring MUST BE obtained before your consultation call.

(STEP 3: Identity Proof Document Upload)

Please upload your Proof of Identity documents below. Required Documents are copy of your ID, and proof of address. A recent utility bill is always the best proof to submit. The credit bureaus need this information to establish your identity and proof of residence. It is imperative that you upload or send us these documents so that it will not delay us in beginning your services.

*Your proof of address document must be no more than 60 Days old.

    (STEP 4: Credit Card Payment Authorization Form)

    To start the credit restoration process, we need to have a payment method on file. Please fill out all fields on this Credit Card Authorization Form. By signing this form you give (Five Star Credit Solutions LLC) permission to charge your debit or credit card for the amount and date indicated in the contract for all credit restoration services. This is permission for all fees stipulated in the agreement, and does not provide authorization for any additional unrelated debits or credits to your account.

    Your initial credit analysis/enrollment fee will be charged after your consultation. If your on-boarding is not completed within 48 hours of the consultation call, then you will be canceled from Five Star Credit Solution's Services. Monthly payments of $99 start a month after your enrollment payment. Your monthly payments are for previous work performed on your behalf. This date will be determined by the date that your first round of disputes went out.

    Credit/Debit Card Authorization Acceptances

    I authorize (Five Star Credit Solutions LLC) to charge the debit or credit card indicated in this authorization form according to the terms outlined above. This payment authorization is for services described in the credit restoration agreement, for the amount indicated in the agreement, and it's valid for the duration of the agreement. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in the agreement.

    Please note: If you do not agree and/or choose not to sign this agreement, Brandon Beckwith reserves the right to revoke the Credit Restoration agreement created by Brandon Beckwith at no fault of Brandon Beckwith. Along with revocation of the agreement, no work will begin on any and all project(s) described in the agreement until this agreement has been agreed upon, signed, and dated.

    **Credit/Debit Card Payments to us for services will appear on your bank/billing statement as "Brandon L. Beckwith Enterprises".

    NOTE: Upon submission of this page you will be redirected to Part 2 of the Enrollment/Onboarding Process which is the final stage “Scheduling your Credit Analysis Consultation”.
